Advocacy
Clients who have memory issues sometimes need an advocate at medical appointments. Spock Care Management can fill that gap by providing an aging life care manager to advocate for client and ensure implementation of medical provider’s recommendations through coordination and communication with client’s support systems.
Assessment
Client’s current support systems and environment sometimes need to be assessed to determine safety and appropriateness. SCM’s aging life care manager can perform an assessment of client’s current living environment, then offer suggestions for next steps and resources that will better ensure client’s safety, with the hope that client can remain in his/her current environment for as long as possible.

Families and clients often are uncertain about steps to take to obtain appropriate care and services for aging family members. Aging life care manager can visit with clients and/or family members on a one-time basis to coach and educate them about next steps. A consultation can include suggestions for safer settings or resources that can provide appropriate supports in the current living environment.
Long-term Care Management
Some clients desire long-term professional care management. If so, aging life care manager can visit clients in their current setting, along with attending medical provider appointments. Aging life care manager can also manage additional services in the home setting, including caregiver services and home maintenance services. On-going care management support would be tailored to the client’s needs and wishes.
